HIV Primary Care

Positive Health is proud to provide treatment and services to who are living with HIV, while providing preventive services to all interested. Patients receive personalized care with access to primary care, mental health, and specialty practices. Your team may include primary-care physicians, nurse practitioners, infectious disease fellows, sub-specialty providers, nurses, and social workers. An infectious disease specialist will lead the team to ensure comprehensive and continuous care. Additionally, patients living with HIV are given the option to enroll in one of our support programs. Patients will then be assigned a highly trained case manager/linkage specialist to help a patient navigate their health care.
