The goal of Positive Health Management is to provide excellent, safe, & culturally competent services to persons who are at risk for or are living with HIV.
Wyckoff Heights Medical Center's Positive Health Management department increases access and offers client-centered and anti-discriminatory high-quality health care and supportive services for all clients accessing care at our clinic. Care is delivered via a one-stop shop model to ensure optimal individual health outcomes and is designed to improve patients' experiences and health care. Patients can come to our health center and expect high-quality HIV/STI prevention and care services, behavioral health services, substance use services, and resource linkage and referrals.
Located at 1610 Dekalb Avenue, Brooklyn, NY 11237, Wyckoff's PlaySure Network 2.0 (PSN 2.0) program is housed in the Positive Health Management (PHM) health center. Our services are open to all while prioritizing the following communities:

Black and Latino men who have sex with Men (MSM),

Youth and young adults ages 13-34,

People who identify as Black & Indigenous People of Color (BIPOC),

People who identify as Lesbian, Gay, Bisexual, or Queer

People who use drugs and/or substances

People with diverse immigration statuses
The majority of our patients are residents of the local neighborhoods that Wyckoff is located in. Our service area includes, but is not limited to, six local zip codes where over 75% of Wyckoff’s patients reside: 11237 (Bushwick), 11206 (Williamsburg), 11207 (East New York), 11208 (Cypress Hills/East New York), 11221 (Bed Stuy), and 11385 (Ridgewood). These neighborhoods are home to one of the most ethnically and culturally diverse communities in New York City and that is represented in our staff and patient population.
